1. Eligibility: To participate in the CDA Presents 2026 Shop & Win! Sweepstakes (the “Sweepstakes”), you must be at least the age of majority in your jurisdiction of primary residence at the time the prize is received and a legal resident of the fifty (50) United States or the District of Columbia. California Dental Association (“Sponsor”), its affiliates, subsidiaries, representatives, consultants, contractors, legal counsel, advertising, public relations, promotional, fulfillment, and marketing agencies, and each of their employees, officers, trustees, and directors (collectively, the “Sweepstakes Entities”), and members of their immediate families (spouses, parents, siblings, and children, regardless of where they live) and those living in the same household, whether or not related, are not eligible to enter or win. Exhibitors and their officers, directors, employees, representatives, and agents are not eligible to enter or win. Participation constitutes participant’s full and unconditional agreement to these Official Rules and to Sponsor’s decisions, which are final and binding in all matters related to the Sweepstakes and which Sponsor may decide in its sole discretion. Winning a prize is contingent upon fulfilling all requirements set forth herein. This is a trade promotion and is not open to the general public.

3. How to Enter: During the Sweepstakes Period, there are two (2) ways to enter. 1) Shop Exhibit Hall: During CDA Presents Anaheim 2026 show hours, go to the exhibit hall located in Anaheim Convention Center, 800 W Katella Ave, Anaheim, CA 92802 and spend a total of $500.00 or more on the products or services of one or more of the exhibitors in the exhibit hall. Visit the Member Benefits Center booth #2238 located in the Anaheim Convention Center and have a Sponsor staff member verify that your total spend is $500.00 or more. Once verified, the Sponsor staff member will scan your badge using a dedicated CSI lead scanner. Upon scanning of your badge, you automatically will be entered with one (1) entry in the Sweepstakes. 2) Email entry: To enter without shopping the exhibit hall, send an email to Sponsor at [email protected] RE: CDA Presents 2026 Shopping Incentive Sweepstakes. Specific information to be included in entry email: your first and last name, and email address.

You must provide all required information to be eligible to enter and win. By entering, you agree that Sponsor may contact you via email. Normal internet access, phone and usage charges imposed by your online or phone service may apply. LIMIT ONE (1) ENTRY PER PERSON, REGARDLESS OF METHOD OF ENTRY. ADDITIONAL ENTRIES MAY BE DISQUALIFIED. Those who have won are unable to enter on another day during the Sweepstakes. Entries must be submitted by the participant. Any attempt by any participant to obtain more than the stated number of entries and/or plays by using multiple and/or different identities, forms, registrations, email addresses, logins, or any other methods will disqualify that participant’s plays and/or entries, and that participant and/or entries may be disqualified. Multiple participants are not permitted to share the same email address. Entries specifying an invalid, non-working, or inactive email address will be disqualified. Incomplete, illegible, corrupted, or untimely entries will be disqualified. All entries become the property of Sponsor.

4. Selection of Winners: On May 14, 2026 and May 15, 2026, Sponsor will select the name of two (2) potential winners of prizes in a random drawing from among all eligible entries received as of that day. On May 16, 2026, Sponsor will select the name of one (1) potential winner of prizes in a random drawing from among all eligible entries received as of that day. The odds of winning are based on the number of eligible entries received as of the day of the drawing. The potential winners will be notified by email within seven (7) days of the drawing. Except where legally prohibited, each potential winner must sign and return, within seven (7) days of being notified, an Affidavit of Eligibility, Liability & Publicity Release in order to claim his/her prize. If a potential winner cannot be contacted via email after the first attempt to contact him/her, or if he/she fails to sign and return the Affidavit of Eligibility, Liability & Publicity Release, or any other documentation that Sponsor may require, in its sole discretion, within the required time period, or in the event that a potential winner is disqualified for any reason, Sponsor will award the applicable prize to an alternate winner selected by a random drawing from among all remaining eligible entries. Only two (2) alternate drawings will be held, after which the prize will remain un-awarded.

Verification of Potential Winners: ALL POTENTIAL WINNERS ARE SUBJECT TO VERIFICATION BY SPONSOR, WHOSE DECISIONS ARE FINAL AND BINDING IN ALL MATTERS RELATED TO THE SWEEPSTAKES. A PARTICIPANT IS NOT A WINNER OF ANY PRIZE UNLESS AND UNTIL PARTICIPANT’S ELIGIBILITY HAS BEEN VERIFIED AND PARTICIPANT HAS BEEN NOTIFIED THAT VERIFICATION IS COMPLETE.

5. Daily Prizes:

May 14, 2026 Drawing – Two (2) Prizes will be awarded.

May 15, 2026 Drawing – Two (2) Prizes will be awarded.

May 16, 2026 Drawing – One (1) Prize will be awarded.

Each Prize consists of one (1) $100 Amazon gift card. Issuer terms and conditions apply to card; see front and back of card for terms. Approximate Retail Value (“ARV”) of each prize: $100.

Prize Restrictions: Any and all applicable federal, state, and local taxes and all fees and expenses related to acceptance and use of prize not specifically stated herein are the responsibility solely of winner. Prize cannot be substituted, assigned, transferred, or redeemed for cash; however, Sponsor reserves the right to make equivalent prize substitutions. Sponsor will not replace any lost or stolen prizes. Prize cannot be used in conjunction with any other promotion or offer. Only the number of prizes stated in these Official Rules is available to be won in the Sweepstakes. If, by reason of a print or other error, more prizes are claimed than the number set forth in these Official Rules, prize winner(s) will be selected in accordance with the winner selection method described above from among all eligible claimants making purportedly valid claims to award the advertised number of prizes available.

6. General Conditions: WARNING: ANY ATTEMPT TO UNDERMINE THE LEGITIMATE OPERATION OF THE SWEEPSTAKES MAY BE A VIOLATION OF CRIMINAL AND CIVIL LAW, AND, SHOULD SUCH AN ATTEMPT BE MADE, SPONSOR RESERVES THE RIGHT TO PROSECUTE AND SEEK DAMAGES FROM ANY SUCH PERSON TO THE FULLEST EXTENT PERMITTED BY LAW. Sponsor’s failure to enforce any term of these Official Rules shall not constitute a waiver of that provision. Neither Sponsor nor anyone acting on its behalf will enter into any communications with any participant regarding this Sweepstakes, except as expressly set forth in these Official Rules. Sponsor reserves the right at its sole discretion to disqualify any individual it finds to be attempting to tamper with or undermine the entry process, and/or the legitimate operation of the Sweepstakes; to violate the Official Rules; or to act in an unsportsmanlike or disruptive manner or with the intent to annoy, abuse, threaten, or harass any other person. Sponsor and its agents are not responsible for (1) any incorrect or inaccurate information, whether caused by participants, printing errors or by any of the equipment or programming associated with or utilized in the Sweepstakes; (2) technical failures of any kind, including, but not limited to malfunctions, interruptions, or disconnections in phone lines or network hardware or software; (3) unauthorized human intervention in any part of the entry process or the Sweepstakes; (4) technical or human error which may occur in the administration of the Sweepstakes or the processing of entries; or (5) any injury or damage to persons or property which may be caused, directly or indirectly, in whole or in part, from participant’s participation in the Sweepstakes or receipt or use or misuse of any prize. If for any reason a participant’s entry is confirmed to have been erroneously deleted, lost, or otherwise destroyed or corrupted, participant’s sole remedy is another entry in the Sweepstakes, provided that if it is not possible to award another entry due to discontinuance of the Sweepstakes, or any part of it, for any reason, Sponsor, at its discretion, may elect to hold a random drawing from among all eligible entries received up to the date of discontinuance for the prize offered herein. If, for any reason, the Sweepstakes is not capable of running as planned due to causes beyond the control of Sponsor, which corrupt or affect the operation, administration, security, fairness, integrity, or proper conduct of this Sweepstakes, Sponsor may, in its sole discretion, void any suspect entries and (a) cancel or terminate the Sweepstakes (or any portion thereof) and award the offered prize at random from among the eligible, nonsuspect entries received up to the time of the impairment or (b) modify or suspend the Sweepstakes to address the impairment and then resume the Sweepstakes in a manner that best conforms to the spirit of these Official Rules.

7. Limitation of Liability and Releases; Publicity: BY PARTICIPATING IN THIS SWEEPSTAKES, PARTICIPANT AGREES THAT SWEEPSTAKES ENTITIES AND THEIR RESPECTIVE PARENT COMPANIES, AFFILIATES, SUBSIDIARIES, REPRESENTATIVES, CONSULTANTS, CONTRACTORS, LEGAL COUNSEL, ADVERTISING, PUBLIC RELATIONS, PROMOTIONAL, FULFILLMENT AND MARKETING, WEBSITE PROVIDERS, WEB MASTERS, AND THEIR RESPECTIVE OFFICERS, DIRECTORS, EMPLOYEES, REPRESENTATIVES, AND AGENTS (THE “RELEASED PARTIES”) WILL HAVE NO LIABILITY WHATSOEVER FOR, AND SHALL BE HELD HARMLESS BY PARTICIPANTS AGAINST, ANY LIABILITY FOR ANY INJURIES, LOSSES OR DAMAGES OF ANY KIND TO PERSONS, INCLUDING PERSONAL INJURY OR DEATH, OR PROPERTY RESULTING IN WHOLE OR IN PART, DIRECTLY OR INDIRECTLY, FROM ACCEPTANCE, POSSESSION, MISUSE, OR USE OF THE PRIZE, ENTRY, OR PARTICIPATION IN THIS SWEEPSTAKES OR IN ANY SWEEPSTAKES-RELATED ACTIVITY, OR ANY CLAIMS BASED ON TRADEMARK INFRINGEMENT, COPYRIGHT INFRINGEMENT, MORAL RIGHTS, PUBLICITY RIGHTS, DEFAMATION OR INVASION OF PRIVACY, OR MERCHANDISE DELIVERY. 8. Disputes. This Sweepstakes is governed by the laws of the United States. Except where prohibited, participant agrees that: (1) any and all disputes, claims and causes of action arising out of or connected with this Sweepstakes or any prize awarded shall be resolved individually, without resort to any form of class action, and exclusively by the United States District Court for the Eastern District of California or the appropriate State Court located in Sacramento County, California; (2) any and all claims, judgments and awards shall be limited to actual out-of-pocket costs incurred, including costs associated with entering this Sweepstakes, but in no event attorneys’ fees; and (3) under no circumstances will participant be permitted to obtain awards for, and participant hereby waives all rights to claim, indirect, punitive, incidental and consequential damages and any other damages, other than for actual out-of-pocket expenses, and any and all rights to have damages multiplied or otherwise increased. All issues and questions concerning the construction, validity, interpretation and enforceability of these Official Rules, or the rights and obligations of the participant and Sponsor in connection with the Sweepstakes, shall be governed by, and construed in accordance with, the laws of the State of California, without giving effect to any choice of law or conflict of law rules, which would cause the application of the laws of any jurisdiction other than the State of California.

9. Privacy: Information collected from participants is subject to the Sponsor’s Privacy Policy, which can be found at http://www.cda.org. Sweepstakes Entities may use participants’ and/or referrals’ personal information submitted with entry for purposes of prize fulfillment and/or for future marketing by Sponsor, such as to notify them of a product or promotion that Sponsor thinks may be of interest.

By participating in this Sweepstakes, participant acknowledges and agrees that he or she will clearly and conspicuously disclose that he or she is participating in this Sweepstakes in the entry and any online social sharing interaction that references the Sweepstakes or that is used to obtain entries in connection with this Sweepstakes. Sponsor reserves the right to disqualify any participant who does not adequately disclose his or her participation in this Sweepstakes.

4. Selection of Winners: On or about March 27, 2026, Sponsor will select the name of one (1) potential winner of prizes in a random drawing from among all eligible entries received. The odds of winning are based on the number of eligible entries received. The potential winner will be notified by email within seven (7) days of the drawing. Except where legally prohibited, the potential winner must sign and return, within seven (7) days of being notified, an Affidavit of Eligibility, Liability & Publicity Release and IRS Form W-9 in order to claim his/her prize. If a potential winner cannot be contacted via email or phone after the first attempt to contact him/her, or if he/she fails to sign and return the Affidavit of Eligibility, Liability & Publicity Release and IRS W-9 Form, or any other documentation that Sponsor may require, in its sole discretion, within the required time period, or in the event that a potential winner is disqualified for any reason, Sponsor will award the applicable prize to an alternate winner selected by a random drawing from among all remaining eligible entries. Only two (2) alternate drawings will be held, after which the prize will remain un-awarded.

5. Prizes: There will be one (1) Prize awarded. The Prize consists of roundtrip coach airfare for Prize winner and up to three (3) guests (each, a “Guest”) from the airport nearest the winner’s residence to John Wayne Airport, Los Angeles International Airport, Long Beach Airport, or Hollywood Burbank Airport. Guests must be at time of prize fulfillment the age of majority in the jurisdiction in which s/he resides (and at least eighteen (18) years of age), unless accompanied by a parent or legal guardian, and must complete and return a liability/publicity release and any other documentation required by Sponsor prior to prize award. If Guest is a minor in his/her state of residence, minor’s parent/legal guardian must complete and return all releases on behalf of minor. Total ARV of Prize: $1500. Travel restrictions, conditions and limitations may apply. ARV of Prize may vary depending on point of departure, travel dates and fare/rate fluctuations. Any person that participates in the Prize that is not deemed to be a Guest must pay his/her own expenses.

Prize winner and his/her Guests must be able to travel between May 11, 2026 to May 18, 2026. All meals, ground transportation and incidentals not described herein and incurred by Prize winner and his/her Guests will be the sole responsibility of the Prize winner and/or his/her Guests.

Travel must be completed by May 18, 2026. Sponsor will determine which airport is closest to the winner’s home. If winner resides within a two hundred fifty (250) mile radius of Anaheim, California (a “local resident”), Sponsor reserves the right to substitute, in lieu of air transportation, a $200 gas gift card and free parking at the Anaheim Convention Center for up to two (2) cars between May 14, 2026 to May 16, 2026 during the hours of 6:30 a.m. – 6 p.m. daily. With respect to the gas gift card, issuer terms and conditions apply to card; see front and back of card for terms. With respect to the free parking, overnight parking is not permitted and restrictions apply. No compensation will be awarded in lieu of the difference in the ARV for the Prize for a local resident and the ARV for the Prize who is not a local resident. Winner and his/her Guests must travel together. Travel subject to availability and blackout dates. Winner and his/her Guests are responsible for obtaining all necessary travel documents. Travel arrangements must be made through Sponsor’s agent and on an airline carrier of Sponsor’s choice by completing and submitting the form to travel coordinator at cda.org. If required, winner and his/her Guests must be available to travel on dates specified by Sponsor or risk forfeiting prize. Certain travel restrictions may apply. If winner elects to travel or participate in the prize travel with fewer than three (3) Guests, no additional compensation will be awarded. If winner elects to travel or participate in prize travel with more than three (3) Guests, such additional persons shall be responsible for any and all costs associated with such travel. Winner and his/her Guests are also responsible for obtaining travel insurance (and all other forms of insurance) at their option and hereby acknowledge that Sponsor has not and will not obtain or provide travel insurance or any other form of insurance. No refund or compensation will be made in the event of the cancellation or delay of any flight except at Sponsor’s discretion. Travel is subject to the terms and conditions set forth in this Sweepstakes, and those set forth by Sponsor’s airline carrier of choice as detailed in the passenger ticket contract. The passenger contracts in use by the airlines or other transportation companies shall constitute the sole contracts with respect to the prize winner’s and his/her Guest’s transportation and such contracts shall be solely between the prize winner, his/her Guest and such transportation companies. Lost, stolen or damaged airline tickets will not be replaced or exchanged. With the exception of stopovers approved in advance at Sponsor’s sole discretion, no stopovers are permitted on tickets issued as part of the prize. If a stopover occurs, the prize travel will terminate and winner and his/her Guests will be responsible for the full fare which will be charged from the stopover point for the remaining trip segment(s), including the return. Sponsor shall not be responsible for any cancellations, delays, diversions or substitutions or any act or omissions whatsoever by any air carrier(s) or other transportation companies or any other persons providing any of these services to passengers including any results thereof such as changes in services or accommodations necessitated by same. Airline tickets issued in connection with the prize may not be eligible for frequent flyer miles or upgrades.

In the event winner and/or his/her Guests engage in behavior during travel that (as determined by Sponsor) is illegal, tortious, or subjects winner to arrest or detention, Sponsor shall have no obligation to pay any damages, fees, fines, judgments or other costs or expenses of any kind whatsoever incurred by winner and/or his/her Guests as a result of such conduct. SPONSOR SHALL HAVE NO LIABILITY FOR ANY PERSONAL INJURIES, DEATH, PROPERTY DAMAGE, OR OTHER DAMAGES OR EXPENSES RESULTING FROM OR ARISING OUT OF ANY TRAVEL RELATED TO THE PRIZE OR ANY OTHER ASPECT OF PRIZE WINNER’S ACCEPTANCE OR USE OF THE PRIZE.

CDA STORY USE DISCLAIMER

I give permission to California Dental Association, its subsidiaries, affiliates, licensees, successors and assigns, and those acting under their authority (collectively, “CDA”) to use, and to authorize others to use, my statement written above and my name and biographical information in connection with that statement (collectively, the “Story”), forever throughout the world. CDA may use, copy, exhibit, publish, and distribute the Story, in whole or in part, in print, television, radio, film, digital media (including internet, social media, websites, and apps), and in all other media now known or hereafter existing for advertising, marketing, publicity, and promotion of CDA and its services and all other commercial business purposes. CDA may modify the Story so long as the original content is not significantly altered or misrepresented.

I understand and acknowledge that CDA (a) will be under no obligation to actually use the Story and (b) has the right to use the Story either with or without my name or biographical information.

I waive the right to inspect or approve any use by CDA of the Story.

I certify that: (a) the Story is true and accurate and reflects my personal experiences and honest beliefs; (b) I made the Story of my own free will; and (c) if I am a health care provider, the Story contains no personally identifiable information, including without limitation, protected health information, of any patient.

I acknowledge that I will receive no compensation from CDA other than any goodwill and publicity that I may receive relating to CDA’s use of the Story.

I hold harmless and release CDA, its officers, directors, agents, and employees from all claims, demands, and liabilities of any kind arising out of or in connection with CDA’s use of the Story, including without limitation, claims based upon invasion of privacy, defamation, copyright or trademark violation, right of publicity, or false light. I will not make any claim against CDA for its use of the Story.

I ACKNOWLEDGE THAT I AM OVER 18 YEARS OF AGE AND READ AND UNDERSTOOD ALL THE TERMS OF THIS RELEASE.
